Claims Procedures

In-Hospital / Cold Case


Adherent after attending physician visit who recommends in-hospital admission for a Medical or Surgical treatment must prepare and do the following:

  • Get the Medical Report for Admission (MRA) proper to GlobeMed Lebanon filled, stamped and signed by the attending physician.
  • Head to the provider admission office to set a date for the admission.
    • If GlobeMed Lebanon has a representative office in the provider premises, Adherent head to this office with the MRA, Medical Insurance card , ID card and Mobile number which are to be provided to the delegate who will prepare for him his admission.
    • If no GlobeMed Lebanon office available at the provider, Adherent must give the MRA as well as the Medical Insurance card, ID card and Mobile number to the admission office that will send the request to the claims center for processing.
  • GlobeMed Lebanon delegates will provide the Hospital with the approval. And in case of rejection adherent will be contacted.

N.B.: If adherent has NSSF coverage, he should get the NSSF approval prior to admission and provide it to the provider.

In-Hospital / Hot Case


This is particular for admission In-Hospital via ER.

  • Adherent should present his Medical Insurance Card as well as his ID Card. Provider will contact GlobeMed Lebanon for approval.
  • Medical Report for Admission filled, stamped and signed by the attending physician should be provided later on by the provider.

NB: If adherent has NSSF coverage, he should get the NSSF approval and provide it to the provider.

ER Case


Adherent should present his Medical Insurance Card as well as his ID Card. Provider will contact GlobeMed Lebanon for approval.

In case of a motor accident, you must follow the below steps


    Call the company on our “HOTLINE” 24h/24h 81600436
  • Advise the company in writing within three days after the accident and fill up a declaration form.
  • The company has to evaluate the cars’ damages [(yours and the third party (ies)] before any repairs are done at the following help points:

  • Do not bear, under any circumstance, any responsibility or any payment on your behalf if not authorized or recognized by the company.
  • In case of non application of the above statements the company will decline your claim.

In case of injury, you must follow the below steps:


  • Fill in the declaration form already received with the insurance policy. The injured to head to the nearest hospital.
  • Provide the company with a copy of the declaration form along with the medical report, prescription and medicines bills.
  • Inform Libano-Suisse when the injured is back to work and/or in case he needs any medical follow-up.
  • Insured to collect from SMBI the rest days indemnity and the medicines bills and to sign a discharge and other legal documents.

In case of fire, you must follow the below steps


  • Take all steps to minimize the extent of loss or damage and save the undamaged items.
  • Notify the insurer in a maximum period of three days by telephone as well as in writing giving an indication to the nature and extent of loss or damage and its first estimation.
  • Preserve the parts affected and make them available for inspection by a surveyor sent by the insurer.

In case of burglary, you must follow the below steps


  • Notify the insurer by telephone as well as in writing in a delay of 24h giving an indication to the nature, extent of loss or damage and its first estimation.
  • Inform the police authorities and advise them about the missing items in order to be mentioned in their report.
  • Preserve the parts affected and make them available for inspection by a surveyor sent by the insurer.
